This study aimed to evaluate efficacy of Percutaneous Transluminal Coronary Angioplasty (PTCA) with stenting for treatment of proximal LAD disease. Materials & Methods: In this descriptive study, 75 patients with isolated proximal LAD atherosclerotic lesion treated with PTCA and stenting were followed up for 9 months. The stenosis of all patients was documented by coronary angiography prior to enrollment. Exercise tolerance testing (ETT) by Bruce protocol was performed before angioplasty and at the end of follow up period as well. Results: There were 44 (58.7%) males, and 31 (41.3%) females. The mean age of patients was 59.4 (range, 39 to 83 yrs). Clinical outcomes of 73 (97.3%) patients were satisfactory. Only two patients developed re-stenosis during the follow up period. The first patient developed myocardial infarction at the 3rd day of her admission, and the second one had an abnormal ETT after 9 months. Percutaneous coronary revascularization with stenting was performed, and both of them were discharged without any problem. Conclusion: This study demostrated that the nine months prognosis of patients with isolated stenosis of proximal LAD coronary artery that treated by PTCA and stenting was acceptable. This treatment improves clinical status and results in lower hospital adverse event rates and in appropriate clinical outcome. چکیده فارسی مقاله زمينه و هدف : ضايعات آترواسکلروتيک شريان کرونری قدامی نزولی چپ (LAD) به دليل خونرسانی اين شريان به بيش از 50درصد از بافت ميوکارد بطن چپ همواره مورد توجه و بحث از نظر درمان مناسب بوده است. اين مطالعه به منظور ارزيابی کارايی آنژيوپلاستی با تعبيه استنت (PTCA) در درمان اين بيماران صورت گرفت. روش بررسی: در اين مطالعه توصيفی آينده‌نگر 75 بيمار (31 مرد و 44 زن) مراجعه کننده به بيمارستان بقيه الله (عج) تهران با درگيری مجزای قسمت پروگزيمال شريان LAD که تحت PTCA قرار گرفته بودند، به مدت 9 ماه از نظر کارايی درمان انجام شده، مورد ارزيابی قرار گرفتند. تنگی آترواسکلروتيک شريان LAD در تمامی بيماران با استفاده از آنژيوگرافی ثبت گرديد. کليه بيماران قبل و 9 ماه بعد از انجام آنژيوپلاستی تحت تست ورزش با استفاده از پروتکل بروس قرار گرفتند. يافته‌ها : ميانگين سنی بيماران 4/59 سال (محدوده 39 تا 83 سال) بود. در 73 بيمار (3/97درصد) آنژيوپلاستی با موفقيت همراه بود. در مدت زمان پيگيری 2 بيمار (7/2درصد) دچار تنگی مجدد شدند که يکی با آنفارکتوس ميوکارد در روز سوم پس از PTCA و ديگری با اختلال در تست ورزش انجام شده 9 ماه بعد از PTCA مشخص گرديد. برای هر دو بيمار استنت‌گذاری مجدد انجام شد و با حال عمومی مناسب ترخيص شدند. نتيجه‌گيری: اين مطالعه نشان داد که پيش‌آگهی 9 ماهه بيماران با درگيری مجزای پروگزيمال شريان LAD که تحت درمان با PTCA قرار گرفته بودند، رضايت‌بخش بود. اين روش درمانی وضعيت بالينی بيماران را بهبود داده و سبب کاهش عوارض ناشی از اقامت طولانی‌مدت بيماران در بيمارستان می‌شود. کلیدواژه‌های انگلیسی مقاله Ischemic heart disease, Left anterior descending coronary artery, Coronary angioplasty, Stent کلیدواژه‌های فارسی مقاله بيماری ايسکميک قلب ، شريان کرونری قدامی نزولی چپ ، آنژيوپلاستی عروق کرونر ، استنت صفحه آغاز مقاله در نشریه ۵۴ صفحه پایان مقاله در نشریه ۵۸ نشانی اینترنتی http://www.goums.ac.ir/journal/browse.php?a_code=A-10-1-305&slc_lang=fa&sid=fa فایل مقاله فایلی برای مقاله ذخیره نشده است نویسندگان مقاله 3841---3842---3843---3844---3845--- برای برگشت به بخش مقاله‌های منتشر شده اینجا را کلیک كنید.
